I use notepad on my mac and input name and date. Copy and paste 4 times for a 5-er. Print the sheet. I cut out them out in 1/4" by 2" or so. I use a glue stick to affix them to the bottom 1/3 of the stick by wrapping the mini-band around itself until tight. Stick with glue stick and presto, no more guessing.

It would be really cool if the CS "Buddy List" could have a small note field for each "buddy".
