Definitely not eating can be a factor. For me it also depends on recent smoking behavior. If I haven't eaten and I haven't had a cigar for a couple of week (shamefully that does happen from time to time) I am much more susceptible to a nic buzz. You want to get laid out try JdN Anato, La Flor Dominica DL lines or LG Diez. Both of those gave me a serious nic buzz. Still it's more often the situation than the cigar. I have all of the ones I just mentioned and didn't feel a thing and I have had medium bodied smokes hit pretty hard because a combination of lack of food and drinking alcohol. The alcohol and nic can interact and reinforce each other.
