I had a Trilogy tri-press that had the same symptom a few months ago. I didn't do an autopsy, so I don't know if it had the same cause, but that collapsed area at the cap was almost identical.

Interesting, I had a CAO Black Storm recently and the taste was definitely sweet grassy, a new vocab in my cigar review lingo. A mild smoke, leaving little on palate.
